Certified Pharmacy Technician (CPhT) certification and 1+ years Pharmacy Technician experience required. Applicants who do not meet these qualifications will not be considered.

This opportunity is ideal for experienced clinical pharmacists with a strong background in home infusion and pharmacy operations. You will play a critical role in ensuring the safe and effective use of medications across multiple infusion sites, working closely with pharmacy technicians and healthcare teams to optimize patient care.

Key Responsibilities :

• Review and interpret prescriptions for verification and accuracy.
• Assess patient records to determine the appropriateness of medication therapy.
• Provide patient medication counseling to ensure understanding and adherence.
• Detect therapeutic inpatibilities and intervene when necessary.
• Oversee and validate pharmacy technician activities and documentation.
• Collaborate with internal departments such as Reimbursement, Quality Management, Nursing, and Clinical Services.
• Manage medication therapy for specialty disease states to improve patient outes.
• Work within multiple infusion site pharmacies, ensuring pliance with clinical protocols.
