RN Manager - Inpatient Care Management

The Manager of Care Management is responsible for the direction, leadership and oversight management for the assigned Care Management departments. This position is designed to facilitate an effective process of the Mercy Care Management Model supporting quality patient care, patient safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; collaborating with facilities to include leadership coordination as evidenced by metrics, optimizing performance and adoption of best practice.

What You’ll Do :

• Oversee Care Management operations, budgets, and metrics.
• Implement performance improvement plans and monitor results.
• Ensure compliance with regulatory standards and accreditation processes.
• Serve as a resource for case management, discharge planning, and utilization review.
