Job description

Physical Therapist Role $48 to $52 per hour

Overview We are seeking a dedicated and compassionate Physical Therapist (PT) to join our healthcare team. The ideal candidate will independently evaluate, plan, and implement comprehensive treatment programs to help patients regain function, mobility, and independence across various settings, including inpatient, outpatient, skilled nursing facilities, and home health. This role requires strong clinical reasoning, assessment skills, and the ability to develop individualized care plans for diverse patient populations.

Duties

  • Perform initial evaluations, establish treatment goals, and create individualized care plans for patients with orthopedic, neurological, geriatric, pediatric, and acute conditions.
  • Provide direct patient care utilizing therapeutic exercises, manual therapy, and other evidence-based techniques to promote recovery and functional improvement.
  • Supervise and delegate tasks to Physical Therapist Assistants and other support staff, ensuring continuity and quality of care.
  • Reassess and adjust treatment plans based on patient progress and changing clinical status.
  • Educate patients and families on home exercise programs, body mechanics, and injury prevention strategies.
  • Collaborate with physicians, nurses, and other members of the interdisciplinary team to optimize patient outcomes.
  • Participate in discharge planning and ensure smooth transitions of care between inpatient and outpatient settings.
  • Maintain documentation in accordance with facility standards and regulatory requirements.

Requirements

  • Current licensure as a Physical Therapist (PT) in the state of practice.
  • Graduate of an accredited Physical Therapy program.
  • Strong understanding of anatomy, physiology, kinesiology, and rehabilitation principles.
  • Experience in skilled nursing or acute care settings preferred.
  • Excellent communication and documentation skills.
  • Demonstrated ability to work independently and as part of a multidisciplinary team.
  • Experience in geriatrics, orthopedics, or neurological rehabilitation is a plus.
