- Monday to Friday schedule no weekends or holidays with guaranteed 40 hours / week
- 25 - 30 patient visits per week based on travel requirements
- Home health setting serving adult population
- EMR : HCHB
- Home health and OASIS experience strongly preferred
- 6+ month commitment with 8 week orientation provided
