About The Position

Job Title : Occupational Therapist

Profession : Occupational Therapist

Shift : Days

Hours per Shift : 8 hours, Monday to Friday (Times are flexible as long as consistent and communicated)

Experience : 1 year of OT experience required; New Grads welcome with Level II experience

License : Occupational Therapist license required

Must-Have :

• Experience in Long-Term Care or Rehab required

Description :

Effectively screens and evaluates patients with cognitive and physical functioning disorders.

Develops appropriate plan of care following regulatory and clinical practice standards.

Utilizes standardized assessments and evidence-based practice to support clinical interventions.

Provides rehabilitative, skilled, and medically necessary treatment to patients with cognitive and physical functioning disorders.

Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, and exercises.

Assesses patient progress and adjusts treatments accordingly in the clinical record.

Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluation.

Provides consultation and counseling to patients, families, caregivers, and other service providers.

Effectively communicates with supervisors and other health team members regarding patient progress.

Documents all required information in the electronic documentation system and maintains patient records according to policy.

Accurately records treatment minutes for services provided.

Participates in required meetings, including patient care conferences and rehabilitation meetings.

Engages in facility and company-required trainings, in-services, and conferences.

Able to obtain necessary durable medical equipment or supplies as required.

Supervises and supports occupational therapist assistants, rehabilitation aides, and students.

Adheres to professional practice acts, code of conduct, and ethics during practice.
