General Statement of Duties
Under direct supervision at the entry level and general supervision of the journey level, performs basic support functions in clinics and community settings and serves as a vital link between communities and health services to promote health among individuals, families and groups who lack access to adequate care and services. Assists in the development and implementation of comprehensive community-based education and prevention programs; processes documents, implements records management, and assists with educational materials and performs other related duties as required. This classification may be flexibly staffed, if allocated to the hiring department as flexibly staffed.
Reports to
Deputy Director of Public Health Education, Public Health Project Coordinator, Deputy Director of Public Health, Women Infant Children (WIC) Program Manager.

Examples of Duties
The essential functions of this position include but are not limited to assisting various Public Health professional and technical staff in providing a wide variety of outreach services in the area of community public health. Makes appointments, keep records, gather data, write reports, and interview clients; assist in developing immediate solutions to emergency problems and expediting delivery of needed services, may conduct limited basic health screening and assessments. Assists in teaching basic health practices to clients and families, reinforcing basic public health concepts of prevention and health care, nutrition, hygiene, oral health, tobacco and drug free activities and related.
Assists in interviewing, gathering and assembling related information and preparing reports; maintains appropriate records and files. Assists staff in working with clients of non-traditional cultural patterns and dealing with various cultural barriers. Participates in development of programs and delivery of services for high-risk groups in the community.
Provides basic community health education to clients, community groups, inmates and interested parties. Explain available community resources to clients; confer with professional staff to assist in evaluating client needs or problems. Assist in developing immediate solutions to emergency problems and expediting delivery of needed services.
May serve on committees or confer with other agencies or departments regarding the needs of the community or of the individual clients. Essential functions include having strong interpersonal skills and the ability to relate to individuals who may not share basic commonality, including value system and behavior norms.

Typical Qualifications
Any combination of training or experience that would provide the knowledge and skills to successfully perform the listed duties is qualifying. A typical way, but not required, to obtain the required knowledge and skills would be :
Three (3) years responsible work experience providing community based health services, working with community based organizations that provide health related services to high risk and culturally diverse groups. Experience in outreach, program administration / planning, public relations, agency coordination and office management.
OR
Completion of an Associate Arts Degree (AA), in public relations, community organization, human services, business administration, public administration, early childhood education.
OR
Licensed Vocational Nursing or related field is desirable.
Education : High school diploma. CPR certification.
Special Requirements
Possession of a valid CA driver's license is required.
Community Outreach Coordinator I assigned to the Women, Infants and Children (WIC) Division will be required to complete the WIC Nutrition Assistant (WNA) training and 1000 hours of experience within 6-12 months after their hire date. Once successfully completed the employee will receive a certificate of competency.
Typical Physical Requirements
Sit for extended periods; frequently stand and walk; stool, kneel and crouch to pick up or move objects; normal manual dexterity and eye-hand coordination; physical ability to lift, push, carry, and move objects weighing up to 25 pounds; corrected hearing and vision to normal range; verbal communication; use of office equipment, including telephone, calculator, copiers, and FAX.
Typical Working Conditions
While performing duties of this position, work is typically performed in a normal office environment. The employee will also be required to travel to all reaches of the county and under varying weather and road conditions. There may also be travel requirements outside of the county for training purposes.
