Organization & Program Summary
Founded in 1965, the Chinese-American Planning Council, Inc. (CPC) aims to promote the social and economic empowerment of Chinese American, immigrant, and low-income communities. CPC offers more than 50 programs at over 35 sites throughout New York City, including early childhood education, school-age care, youth services, workforce development, community services, and senior services.
Asian Family Services (AFS) provides child welfare preventive services to help keep children and families together and avoid foster care placement, addressing multi-level needs of Asian-American families, especially recent Chinese immigrants, through support, advocacy, referral to services, and case management.
Job Summary
Reporting to the Director of Asian Family Services, the Family Team Conference Coordinator will facilitate collaboration between families, the agency, and the Administration for Children's Services (ACS). The Coordinator will coordinate and facilitate Family Team Conferences, ensure effective communication between the agency and ACS, and support timely documentation and follow-up. The Coordinator may carry a caseload as needed, assessing safety and risk while helping families navigate supportive services.
