St. Agnes Hospital is seeking a dedicated Registered Nurse - Case Manager to drive patient-centered care and optimize discharge planning for our patients.

About the Role :

The RN-Case Manager plays a crucial role in assessing, planning, and implementing discharge plans tailored to the needs of patients in collaboration with our interdisciplinary team. This role focuses on ensuring smooth transitions to the next level of care, promoting patient safety, and enhancing overall health outcomes.

Main Responsibilities :

• Ensure adherence to discharge planning regulations and improve overall patient care coordination.
• Identify high-risk patients, analyze the causes for readmissions, and collaborate with the team to create effective discharge plans.
• Participate in initiatives to refine processes and improve team performance.
• May coordinate with post-acute providers such as skilled nursing facilities, rehab centers, and home health services.
• Utilize evidence-based practices to deliver age-appropriate and clinically appropriate care.
• Comply with the established professional nursing standards and the ANA Code of Ethics.
• Maintain a safe and alert working environment.
• Complete other related duties as assigned.
