Job descriptionBe interested in heart failure care Have Cardiology / Heart Failure experience (preferred but not required) Hold an Advanced Practice Nurse or Physician Assistant CT state license. Trinity Health Of New England seeks a Nurse Practitioner or Physician Assistant to join the Cardiology Heart Failure team at Saint Francis Hospital and Medical Center.
RESPONSIBILITIES :
- Seeing patients admitted with heart failure in consultation with the cardiology team
- Performing advanced heart failure consultations
- Working with the interdisciplinary team made up of hospitalists, social workers, nurses, Cardiologists
- Working as an extension of the physician in managing patients with heart failure.
- Skillset includes : Management of diuretics, Guideline directed medical therapy initiation, Management of cardiogenic shock
- Attending interdisciplinary rounds daily
