Behavioral Health Specialist – Contract

Location : Elma, NY

Schedule : ~20 hours / week | Flexible hybrid (in-person / remote)

Position Details

Premise Health is seeking a Behavioral Health Specialist (LPCC, LMFT, LISW, or LCSW) to provide therapy services at an integrated primary care clinic in Elma, NY. This is a 13-week contract with the possibility of extension.

Contract Dates : November 3, 2025 – January 30, 2026

Schedule : ~20 hours per week

Preferred : Two 8-hour days and one 4-hour day

Hours : 7 : 30 AM – 4 : 30 PM (with some flexibility)

Hybrid preferred, open to fully in-person or fully remote

Responsibilities

Provide individual, couples and child / teen therapy

Deliver compassionate, evidence-based behavioral health services in a collaborative primary care setting

Work with patients to develop treatment goals and strategies that support overall health and wellness

Qualifications

Active, unrestricted license in the State of New York (LPCC, LMFT, LISW, or LCSW required)

Experience providing therapy across diverse patient populations

Strong communication and clinical documentation skills

Ability to work independently and as part of an integrated healthcare team
