We are seeking a dedicated Family Nurse Practitioner to join our healthcare team in Eau Claire and Rice Lake. The ideal candidate will provide comprehensive primary care services to geriatric patients and psychiatric hospital coverage, focusing on health promotion and disease prevention.

DUTIES :

• Conduct physical examinations and patient assessments
• Diagnose and treat common acute illnesses and injuries
• Identify chronic health conditions such as diabetes, hypertension, and asthma
• Perform Admission History & Physical
• Educate patients on health maintenance and disease prevention
• Perform Consult and discharge medication reconciliation
• Provide behavioral health support
• Provide primary care services directly in residents homes within assisted living, memory care, and independent senior living communities

JOB TYPE : Full-Time, travel nursing to assisted living facilities in the Eau Claire and Rice Lake

PAY : $140,000.00 annual salary

EXPECTED HOURS : 40 per week

BENEFITS : Two weeks (80 hours) PTO to start, Health Insurance stipend, travel reimbursement
