Pharmacist II
JOB SUMMARY
- Under the direction of the Clinical Coordinator or Manager and the Pharmacy Director, optimizes patient medications therapy by providing comprehensive clinical pharmacy services. Works collaboratively with the health care team to accomplish goals.

Minimum Education
B.S. Pharmacy or Pharm D, or Master of PharmacyContinuing education as required for licensure by TSBPMinimum Work Experience
Previous hospital experience with clinical responsibilitiesRequired Licenses / Certifications
Active registration as a licensed pharmacist in the state of TexasSterile Compounding CertificationBLS, ACLS, or PALS, depending on practice areaRegistration as a preceptor with TSBPRequired Skills, Knowledge, and Abilities
Assess, document, and perform physician consultations, excluding total parenteral nutrition, but including all medication dosing consultsPerform home medication reconciliationProvide education presentations to pharmacy staff and other staff upon requestAbility to effectively direct and / or supervise personnel as appropriateAbility to handle confidential informationAbility to maintain professional appearance and decorum at all timesAbility to demonstrate excellent oral and written communication skillsPIf6d23b5f1edf-30511-37033220
