Job Description & Requirements RN Case Manager StartDate : 1 / 5 / 2026 Pay Rate : $1800.00 - $2700.00 POSITION SUMMARY – RN Case Manager POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Floating among sister campuses required MINIMUM REQUIRED QUALIFICATIONS – FL / compact RN license; BLS; 2 years acute hospital case management experience LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – Monday-Friday 8a-4 : 30p (40hrs / wk) SYSTEMS – Epic START DATE – 1 / 5 / 2026 Facility Location The gateway to the popular vacation destination of Southwest Florida, Fort Myers boasts miles of pristine sandy beaches and unspoiled coastlines.
