$3,000 Sign-On Bonus

POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.

ESSENTIAL FUNCTIONS

• Assess patient’s physical condition according to Plan of Care; determine and / or revise plan of care regarding medications and DME.
• Case manage total care of the patient by coordinating with team members / physicians, supervising C N A activities, and participating in staff and Interdisciplinary Team meetings.
• Complete daily paperwork and charting, thoroughly and accurately according to Policy.
• Participate in on-call rotation.
• May assist with admissions
