$3,000 Sign-On Bonus
POSITION SUMMARY
Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.
ESSENTIAL FUNCTIONS
- Assess patient’s physical condition according to Plan of Care; determine and / or revise plan of care regarding medications and DME.
- Case manage total care of the patient by coordinating with team members / physicians, supervising C N A activities, and participating in staff and Interdisciplinary Team meetings.
- Complete daily paperwork and charting, thoroughly and accurately according to Policy.
- Participate in on-call rotation.
- May assist with admissions
QUALIFICATIONS
Registered Nurse in the state of TexasHold a valid driver’s license and have a dependable means of transportation1 year of nursing experienceCase Management experience (hospice)2+ years of experience in hospice case management strongly preferredProficient skill using a computer is required; Microsoft office suite experience is preferred.BENEFITS
