Salary : $59,748 / yr
Join NuHealth at Nassau University Medical Center, Long Island’s only public safety net hospital and Level I Trauma Center. We are seeking a full-time Medical Social Worker to advocate for LGBTQI+ patients, providing health education, care coordination, and mental health counseling to ensure inclusive, comprehensive care.
In this role, you will :
- Identify and connect LGBTQI+ patients to health services, coordinating care across specialties.
- Develop culturally competent materials and lead outreach to address LGBTQI+ health needs.
- Provide short-term mental health counseling and crisis intervention for LGBTQI+ patients.
- Educate hospital staff on LGBTQI+ cultural competence and inclusive care practices.
- Track outcomes, contribute to program improvement, and support grant reporting.
