Required Qualifications
Occupational Therapist, Skilled
Licenses: OT-WA
Certifications: BLS(Copy Needed)
SSN Required
DOB Required
References: 1 Reference in entire work history

License and SNF experience are required

Preferred Qualifications
Travel experience is preferred
