Job Summary
Will be a Licensed Practical Nurse performing duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients. The Nurse Navigator, functioning in the spirit of teamwork, is accountable for the delivery of care utilizing skills learned in both the clinic and didactic arena. Assists other team members on the unit. The nurse navigator role provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.
Job Duties
Hours : This position is 40 hours weekly. This is a night position. 40 hours per week. Hours will fall between 7 : 00 p.m.- 7 : 30 a.m. Shifts may be 12, 8, 6, or 4 hours in length. Schedules are made based on patient needs.
Weekends and Holidays : May be required based on staffing needs
Travel : Less than 25%- Travel may be required for orientation / training, meetings, or other business needs.
Experience : A minimum of 2 years previous LPN work experience is required. Clinic and Pediatrics experience is preferred. Proficient computer skills. Proficient with Microsoft products such as Word, Excel, PowerPoint, SharePoint, Windows, & TEAMs.
