Qualified Professional
We are seeking a dedicated Qualified Professional (QP) to provide clinical oversight, develop individualized support plans, and coordinate services for adults with I / DD in the Raleigh area. As a QP, you'll collaborate with multidisciplinary teams, families, employers, MCOs, and community partners to ensure high-quality care aligned with NC Division of Mental Health, Developmental Disabilities, and Substance Abuse Services (DMH / DD / SAS) standards.
Key Responsibilities
- Develop plans tailored to individual needs, strengths, and goals.
- Provide support and oversight to others working in the department
- Ensure compliance with Medicaid Waiver requirements and state regulations.
- Monitor service delivery, track progress through data collection, and prepare reports for funding sources and regulatory bodies.
Qualifications Required :
Bachelor's degree in Human Services, Social Work, Psychology, Special Education, or a related field. Current Qualified Professional (QP) certification through the NC Department of Health and Human Services (DHHS) or eligibility to obtain within 30 days of hire.At least 2 years of direct experience working with individuals with I / DD, including case management or clinical support.Knowledge of the NC system of care.Excellent communication, organizational, and problem-solving skills; proficiency in Microsoft Office and electronic health records (EHR) systems.Valid NC driver's license, reliable transportation, and ability to pass background checks (criminal, sex offender, and driving record).Powered by JazzHR
