Registered Nurse (RN) Part Time or Full Time in Zanesville, Ohio

We are looking for a Full-time or Part time RN to join our growing home care services who is experienced in nursing, with emphasis on community health education / experience to meet the needs of individuals and families within their home and communities. The position is flexible and you will be able to control your schedule.\n\nJob Functions : \n

• Complete initial home health assessment of patient and family to determine home care needs.\n
• Conducts home visits as assigned, assessing the patients condition, progress toward goals, providing patients care, teaching and other skilled interventions\n
• Supervisory visits\n
• Supervising LPN and Home Health Aides\n
• Initiates the plan of care and makes necessary revisions as patient status and needs change.\n
• Counsels the patient and family in meeting nursing and related needs\n
• Provide health care instructions to the patient as appropriate per assessment and plan of care\n
• Prepare clinical notes and updates\n
• Clinical Record Review\n
• Take call in a call rotation.\n
• Must have 1 year of nursing experience, reliable transportation.\n\nAcute Nursing is an EOE.\n\nWe offer competitive wages and benefits. Pay discussed during interview.
