Job Coach

STARS - Scottsdale Training and Rehabilitation Services is seeking a Job Coach to join our team!

POSITION PURPOSE :

Responsible for training and coaching of employment skills to clients with special needs working independently in the community. Provide supervision / training of clients to achieve their individual employment goals. Conducts in-person visits, follows up with employee's management, maintains accurate documentation of participant progress, and communicates with clients, guardians, and program management regarding client's work progress. Handle special scheduling needs; ensures completeness, accuracy, and security of participant status and pertinent record keeping in compliance with the Division of Developmental Disabilities (DDD).

DUTIES AND RESPONSIBILITIES :

• Responsible for monitoring participants' time and attendance; maintain documentation for participant files including required records for compliance.
• Report to the case manager the development, challenges, and pace of the participant's growth. Act as liaison for Division of Development Disabilities (DDD) requests. Provide support and follow-up on communications to participants, as well as parents.
• Develop monthly reports as case notes; keep up-to-date training records of participants.
• Serves as liaison between STARS and the client's employer.
• Assist in intake and initial evaluation of persons referred to and accepted by STARS in the respective area and departments; including evaluation of medical records to determine transferability, creation of proper schedule, and assignment prior to job placement.
• Monitor the assigned performance of participants and provide the appropriate support if experience on-the-job difficulties or require more guidance.
• Develop, implement, and evaluate Individualized Program Plans.
• Teach individuals with various mental and / or physical challenges, individually supervise participants in the program, and train on job-related duties
• Ensure the safety and well-being of consumers adhere to OSHA Standards.
