Job description

Job Title : LPN (Licensed Practical Nurse) Home Health

Location : Walla Walla, WA 99362

Shift : Days 5x8 Hour

Start Date : 12 / 01 / 2025

Position Summary :

  • The LPN Home Health Nurse provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse or physician.
  • This role includes monitoring patient conditions, administering prescribed treatments, maintaining accurate documentation in Home Care Home Base (HCHB) , and ensuring quality, compassionate care that supports patient recovery and independence.

Key Responsibilities :

  • Provide skilled nursing care to patients in their homes under RN or physician supervision.
  • Monitor and record vital signs, symptoms, and patient progress.
  • Communicate effectively with RNs, physicians, therapists, and other healthcare professionals.
  • Participate in interdisciplinary team meetings and case conferences.
  • Follow individualized care plans and update as directed by supervising RN.
  • Follow infection control and safety protocols at all times.
  • Maintain patient confidentiality in compliance with HIPAA regulations.

    • Required Qualifications :

  • BLS
  • LPN license in hand
  • 2 Years Of Home Health Experience
