Founded in 1921, Straub Medical Center includes a 159-bed hospital in Honolulu, a network of neighborhood clinics and a visiting specialist program that reaches throughout the state of Hawai'i. Under the general direction of the Manager of Complex Care Management Services or designee, provides a wide variety of duties to engage, support, advocate, and educate community members in overall health and wellness. Serves as a liaison between health and social services in the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, social support, and coordination. Works closely with Complex Care team and social service agencies to provide short-term care coordination and connection to resources and support to programs for clients to improve their health and general well-being through educations and provision of coordination or care and services. Works in both clinical and community-based settings, including client's homes.
