About the Role

We are seeking a Director of Case Management to join our team.

Responsibilities

• Responsible for monitoring quality patient standards, patient and payor satisfaction levels, and for ensuring that patients are receiving prioritized, cost-appropriate and quality services.
• Will assess, organize, develop, implement and evaluate the case management program and activities in accordance with current federal, state and local standards, guidelines and regulations and the Joint Commission standards.
• Demonstrates knowledge and skills necessary to provide case management services to the patient population experiencing medically complex conditions, general medical-surgical conditions with rehabilitation and wound care needs.

Pay range and compensation package

Posted Total Compensation (California Colorado) : The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs. It’s not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $135,000 to $175,000 per year.
