Health Advocates play an essential role on the interdisciplinary care team and are responsible for telephonic outreach to members to inquire about current state health condition. Health Advocates will review available data and coordinate care with the clinical team as members' condition change. Health Advocates assist members with health care appointment scheduling and locating local resources to assist with member non-clinical needs. Additional responsibilities include outreach to members to inform them of the value participating in the Kidney Services program will provide them and scheduling outreach appointments with the clinical team using a scheduling tool.

This role is a fully remote position.
