This service is carried out where the patient calls home : personal home, ALF o Day Shift Office open 8-4 : 30 Monday-Friday (Nurse on call 24 / 7) o Night Shift - NA Patient Ratios 6 visit equivalents per day ( types of visits are weighted differently on a point system) Type of equipment EMR - Cerner Typical hiring profile Skill Set Most recruited for : Home Health Must have : candidates without these skills will not be considered for the role. Experience in completing OASIS Assessments Home Health experience Preferred or nice to have : candidates with these skills will be considered first. Some knowledge of new payment model coming 1-1-20 (PDGM) List typical procedures performed on unit(s) : Comprehensive assessments / evaluations, treatment and care planning. Oversight of PTA s. Patient population : mainly geriatric; post-acute treatment of THA / TKA patients; treatment of patients with chronic illnesses and multiple comorbidities. Best personality Fit : Someone who is easy going, likes to meet people and doesn t mind visiting people in their own homes. Needs to be accepting of others and friendly. REQUIRED CERTIFICATIONS : BLS / CPR certification. Needs current NC license to practice Physical Therapy Floating Area(s) : none Dress Code : Not required to wear scrubs.
