Position Details :

Field-based Home Health PT role

servicing Baldwin Park and surrounding areas

Home Health experience preferred , but willing to consider candidates without prior HH experience

CA Physical Therapy License preferred

– candidates with pending CA license must have applied prior to submission

OASIS experience is required

Guaranteed hours based on completed visits :

25 completed visits = 40 hours per week

Hours guaranteed if 25 or more visits completed weekly

COVID exemptions permitted

Mileage reimbursement

at IRS standard rates between patient visits
