This position offers a sign on incentive of $5,000

LOCATION

655 East Cedar Avenue, Gladwin, MI 48624

SCHEDULE

This is a full time, hybrid position, 40 hours per week. The schedule is flexible and can be Monday - Friday 7:30am - 4:30pm or Monday - Friday 8am - 5pm.

COMPENSATION & BENEFITS

Salary: $55,016 - $74,734 depending on experience

JOB SUMMARYGeneral Description: Responsible for participating in an interdisciplinary assessment regarding individuals seeking or residing in nursing home level of care to determine the least restrictive living environment to meet their overall needs.

Responsibilities and Duties: Assesses OBRA/PASARR referrals assigned by OBRA Coordinator and/or designee. Provides education as needed on the OBRA/PASARR process to community partners, consumers, and families. Completes timely assessments for consumers seeking nursing facility placement or already in nursing facility placements. Ensures consumer placed in appropriate and least restrictive living environment to meet their mental and physical health needs. Provides back-up coverage for OBRA therapists in completing Level II OBRA screens for team leave requests. Ensure Specialized Mental Health services are being provided as authorized within our six-county catchment area. Has an acute knowledge of the Level of Care Determination (LOCD) requirements for nursing facility placements. Ensures compliance with MDHHS-OBRA regulations concerning timelines for all assessments. Completes reviews and delivery of Level II evaluations to the individual, legal guardian, hospital, and nursing facility as required by MDHHS-OBRA. Attends OBRA related team meetings and/or trainings related to professional development and or improving practices. Participates in peer reviews or interdisciplinary meetings prior to submission of evaluations.

EMPLOYMENT STANDARDS

Education: Master's Degree is required with a major in Social Work, Counseling or Clinical/Counseling Psychology.
Job-Related Experience: Sufficient experience to perform the position at a satisfactory level required. Experience with relevant population in a community mental health setting preferred.

Licenses and Certifications: Michigan state licensure. Valid driver's license while operating vehicle on agency business.

CONDITIONS OF EMPLOYMENT

Knowledge: Knowledge of Medicaid Guidelines, OBRA rules and regulations, PASARR, and Level of Care Guidelines. Knowledge of eligibility standards and an understanding of service programs, purposes and elements; knowledge of entitlement programs, eligibility requirements and benefits, use of community resources and services. Knowledgeable about types and levels of available community housing options, including but not limited to nursing facilities, adult foster care, assisted living, and or private homes and what community resources are available to support those in private homes. Knowledge of developmental disabilities, mental illness, and neurocognitive disorders.

Skills: Provides various modes of treatment to individuals residing in or seeking nursing home placements. Keeps abreast of current research literature and applies to practical situations with recipients. Computer experience with windows environment to include clinical software. Ability to use personal computer-based data entry and use computer office applications. Telephone and communication skills.

Abilities: Excellent assessment and diagnostic skills in behavioral health areas. Ability to plan, organize, and complete work in a timely fashion. Able to prioritize and manage time in order to meet OBRA timelines and the needs of the consumer. Excellent ability to communicate both verbally and in writing. Ability to write a comprehensive assessment addressing the individuals served progress, diagnosis, level of care and treatment needs. Ability to establish a relationship to provide, as necessary, guidance, counsel, and direction to the individuals served. Ability to work with fellow staff, individuals served, and other agency professionals on an on-going basis. Demonstrates the ability to present one's self, the agency and the individuals we serve in a positive manner at all times.

Physical Demands: Work is generally sedentary. Travel within CMHCM catchment is necessary.

Work Environment: Work is performed in community settings and may involve dealing with people under very trying or difficult circumstances. Additional work may be completed in an office setting.
