Job descriptionConduct in-home visits, health assessments, and care planning for members with acute and chronic conditions Collaborate with other healthcare providers to deliver high-quality, patient-centered care Participate in call rotation of telephonic on call coverage after hours Bachelors in nursing with Illinois APRN license (Masters preferred). Board certification for Nurse Practitioners and DEA licensure. Valid drivers license and proof of insurance.

This position will serve the Northern Cook County area including Schaumburg, Glenview, Niles and the surrounding areas
Schedule : 1 day per week
$125 per completed visit
