An exciting opportunity is available for a Full-Time Physical Therapist (PT) to join our team at a Skilled Nursing Facility (SNF) in Monticello, Minnesota. This role is ideal for both new graduates and experienced professionals looking to grow their careers in a supportive, flexible, and dynamic environment. New graduates are welcome to apply!

This position offers a variety of career growth opportunities, including professional mentorship, unlimited Continuing Education Units (CEUs), and the chance to work in a network with over 100 facilities across 11 states.

Key Responsibilities:

• Patient Assessments & Treatments: Assess and treat patients based on individualized care plans to meet their unique needs.
• Documentation: Accurately document patient progress, treatments, and outcomes in a timely manner.
• Program Development: Plan, market, and develop new programs to enhance patient care and improve overall outcomes.
• Effective Communication: Collaborate with patients, families, physicians, and healthcare professionals to ensure coordinated care.
• Staff Education: Lead in-service training sessions for fellow therapists and staff.
• Clinic Operations: Assist with the general operations of the therapy department, maintaining high standards of care and efficiency.

Qualifications:

• Licensure: Current Physical Therapist (PT) licensure in Minnesota (or eligibility to obtain licensure).

• Teamwork: Ability to work independently as well as part of a collaborative healthcare team.
