Registered Nurse (RN)

LOCATION : East Brunswick and Central New Jersey

Position Description :

• Plan and provide clinical support to In-Home / Residential patients
• Provide and coordinate patient care, educate patients
• New client intake
• Educate and train staff on all required medical trainings
• Document client care services by charting in client records
• Administer and audit medications and medication administration records
• Perform wellness checks as needed
• Coordinate with client's interdisciplinary team, including maintaining a cooperative relationship among healthcare teams by communicating information; responding to requests and building rapport
• Ability to be flexible and adjust to the needs of the program
• Attend interdisciplinary team (IDT) meetings
• Experience working with individuals with developmental disabilities preferred
• Position Requirements :

Valid New Jersey Registered Nurse License

Valid New Jersey Driver's License

Push, pull, and lift up to 50lbs
