As an Occupational Therapist (OT), you will provide skilled occupational therapy to patients on an intermittent basis at their place of residence. Care is provided according to physician orders and individualized plans of care, under the supervision of the Director of Clinical Services / Clinical Manager.

Responsibilities : Conduct initial evaluations and develop individualized plans of careProvide skilled occupational therapy and perform all prescribed proceduresReassess patient progress every 30 days and update care plans as neededConsult with physicians regarding treatment changes and progressInstruct patients and families in home exercises and functional programsSupervise Certified Occupational Therapy Assistants and Home Health Aides as assignedParticipate in patient care planning with other home health staffDocument visits, progress notes, and discharge summaries accurately and timelyPresent in-service training to agency staff and participate in QAPI activitiesQualifications : Graduate of an accredited Occupational Therapy program and certified by the National Board for Certification in Occupational Therapy (NBCOT) or eligible for National Registration ExaminationLicensed in Ohio (or relevant practicing state)Minimum of 2 years’ experience preferredStrong communication skills and ability to work independentlyCommitment to improving patient outcomes and quality of careWorking Environment : Travel to patient homes within Butler CountyWork indoors in office and patient homesPhysical activity including standing for extended periods and moderate lifting may be requiredSupervision : Reports to : Director of Clinical Services / Clinical ManagerSupervises : Certified Occupational Therapy Assistants and Home Health Aides as assignedMake a difference every day!
