Job description

The Wound Care LPN

Licensed Practical / Vocational Nurse is responsible for assigned nursing care activities including wound care for patients of all ages and other clinical activities associated with safe patient care. The LPN assists with patient and wound care data collection for outcome based care management.

The LPN demonstrates the knowledge and skills necessary to provide care to patients served on his / her assigned unit as described in unit-based competencies, policies, and procedures.

In collaboration with other staff members, the LPN provides bedside patient care, implementing the interventions identified in the plan of care, as directed by the RN.

The Clinic LPN reports directly to the Director of Clinic Operations.

QUALIFICATIONS

Education : Graduate of an accredited Practical School of Nursing.
Licensure / Certification : Current licensure by Missouri State Board of Nursing. BLS certification required within 6 months of employment.
Work Experience : Successful completion of orientation for LPN.

RESPONSIBILITIES / JOB DUTIES

Ensure appropriate patient care documentation.

Implement and assure compliance with wound center and hospital policies and procedures.

Maintain familiarity with regulatory standards.

Report quality of care issues to staff RN or Program Director.

Ensure availability of necessary equipment and supplies.

Communicate with all team members regarding therapeutic interventions.

Perform other duties as assigned.

RN / GN Wound Care

Job title : RN / GN Wound Care $28.00 - $41.34. Job code : 5565 / 5560. Job hours : Various Shifts. The RN / GN is responsible for coordination of clinical activities of the Wound Care Center. These include utilization of skills in organization, planning, implementing, evaluating, and providing care through the use of hospital and nursing standards.

The RN / GN reports directly to the Wound Care Program Director.

QUALIFICATIONS

Education : RN graduate of Accredited School of Nursing.

Licensure / Certification : Must have valid, current license by the Missouri State Board of Nursing. Current Basic Life Support (BLS) certification / Cardiopulmonary Resuscitation (CPR) required.

Work Experience : Bachelors degree (B.A.) from a four-year college or university; or one to two years related experience and / or training, or equivalent combination of education and experience.

RESPONSIBILITIES / JOB DUTIES

Provides case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient / caregiver education related to wound care management, and acts as patient advocate in delivery and coordination of patient care.

Provides efficient and effective care with guidance of the physician during clinic sessions, attends the clinics where scheduled, and is dependable. Able to manage a number of patients at the same time with high energy and organization.

Collaborates with other health care providers, wound care center Physicians, Program Director, and Medical Director regarding clinic and patient needs.

Other duties as assigned.
