Job Title : Home Care Supervisor

Position Summary :
The Home Care Supervisor is responsible for the daily operations of the Home Care team and the clinical and administrative supervision of the Case Managers and other Home Care staff as assigned. They participate in quality improvement activities and marketing activities according to Agency plans and guidelines. They provide overall supervision of client cases in the State Home Care Department. The ideal candidate will have the ability to work closely as part of an interdisciplinary team to assess for program eligibility, evaluate consumer and caregiver needs, and think creatively in order to guide the development and implementation of comprehensive service plans for consumers. Should have a strong working knowledge of community resources.
Job Responsibilities and Performance Standards :
- Participate in meetings with Executive Office of Elder Affairs (EOEA).
- Supervise the daily operations of the Home Care team according to Agency guidelines.
- Conduct interdisciplinary case conferences according to Agency guidelines.
- Participate in quality improvement activities, including but not limited to on-site vendor monitoring, staff satisfaction surveys, and review of all new consumer records according to Agency guidelines.
- Collaborate with colleagues, both internal and external, to identify, develop and / or implement processes, policies, and procedures to best address departmental needs and compliance standards.
- Provide education about clinical issues, programs, services and resources to Home Care staff.
- Provide individual or group supervision for Case Managers and other Home Care staff as assigned according to Agency guidelines.
- Conduct timely performance reviews of Case Managers and other Home Care staff as assigned.
- Participate in the hiring and training process for new case management staff and will participate in monthly departmental meetings.
- Maintain documentation in an accurate, complete, confidential, and timely manner, according to state, federal, and Agency regulations and guidelines.
- Participate in agency marketing activities according to marketing plan. Help to identify potential community partnerships and referral opportunities and will work to build and manage these relationships in order to increase community awareness of agency programs and increase departmental referrals.
- Provide coverage for others as needed according to Agency guidelines.
- Comply with HIPAA and other state and Agency confidentiality requirements.
- If bilingual, provide interpretation and translation to Agency staff as needed.

Bachelor’s degree.Ability to work independently and collaboratively with diverse populations.Exceptional time management and organizational proficiency.Must be knowledgeable of and comfortable using Adobe and MS Word, Excel, SharePoint, and other Microsoft 365 platforms. Experience with Wellsky a plus.Must meet both CORI / SORI and public health screening requirements.Physical Environment :
