Full Time, day shift position in the Pharmacy.
JOB SUMMARY :
This position will review and verify chemotherapy regimens and oversee chemotherapy admixtures. An Oncology Clinical Pharmacist collaborates with and serves as a resource for nurses, physicians and other clinicians who participate in the management of the oncology / hematology patient. Assures optimal drug therapy for the patients. When not doing chemotherapy duties, this pharmacist will serve the department by performing traditional hospital pharmacist functions. The Pharmacist is responsible for coordinating and delivering pharmacy services in compliance with all applicable policies, regulatory agency requirements and state and federal laws.

ESSENTIAL FUNCTIONS :
1. Reviews and monitors patient treatment (chemotherapy and ancillary) medications for appropriateness based on current clinical and institutional guidelines as well as patient-specific criteria.
2. Monitors patients medication regimens for drugs interactions, toxicities, and cost-effectiveness.
3. Assesses all medication orders to ensure they meet established standards and practice guidelines ofAshe Memorial Hospital.
4. Reports order discrepancies to appropriate staff promptly.
5. Advises clinicians on drug selection based on current clinical guidelines to optimize drug therapy.
6. Oversees the safe handling and preparation of various medications, including IV chemotherapy, IV antiemetics, IV steroids, and investigational chemotherapy drugs for outpatient and inpatient use.
7. Checks admixtures for accurate preparation, including the additive(s), parent solution, and label.
8. In conjunction with the Senior Attending Technician, oversees technician activities and performs quality control checks.
9. Maintains current patient information files and records in the pharmacy department tomaintain and support safe and consistent drug administration.
10. Adheres to safety requirements in the handling and preparation of chemotherapy and other hazardous agents according to current guidelines (USP 800, ASHP, NIOSH).
Wears PPE (personal protective equipment) as required by USP 797 and USP 800.
11. Oversees the ordering, storage and handling of medications and supplies.
12. Participates in education activities such as inservices.
13. Answers drug information questions.
14. Participates in the Safety Reporting System.Identifies and reports therapy errors and adverse drug reactions.
15. Participates in the training, education and mentoring of other pharmacists, technicians, and students.
16. Participate in area specific staff and department meetings as necessary.
17. Participates in quality, safety and other project work outside of regular staffing duties.
18. May participate in research activities with publication or presentation at local and / or national meetings.
19. Assists in the development of policy and procedures.
20. May educate patients and their families to ensure their understanding of the goal, administration schedule and toxicity management of chemotherapy treatment and documents the activity and interventions in the medical record.
21. Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient profiles.
22. Prepares and dispenses medications, including sterile non hazardous and hazardous, safely and accurately using appropriate techniques.
23. Monitors medication therapy regimens for contraindication, drug-drug interactions, drug-food interactions, drug laboratory test interactions, allergies, appropriateness of drug and dose and therapeutic duplications.
24. Issues controlled substances to patient care areas and maintains records as required by law.
25. Reviews / interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.
26. Provides clinical consultations and clarification to practitioners. Recommends evidence-based medication therapy regimens and monitoring plans. Suggests appropriate, cost effective therapeutic alternatives to medication staff, as needed.
27. Performs other duties as assigned.
