PRIMARY RESPONSIBILITIES :

The key objectives of the program are to :

1. Provide proactive case management and intervention services for those referred. This includes active outreach and locating those referred.

2. Engage with those incarcerated by Everett Police and provide intervention services to break the cycle of arrest.

3. Provide the client with a single point of contact for connection to multiple health and social services.

4. Maintain active contact with participants who are engaged in services.

5. Develop relationships with local suppliers and community organizations and function as an integral part of the local community.

6. Work to remove barriers that prevent individuals’ access to care, such as transportation needs.

7. Care coordination, including scheduling appointments, arranging transportation, conducting appointment reminder calls, and following up to verify service initiation, member progress, and need for service adjustment and incorporation into the care plan.

8. Track data and outcome measures.

9. Actively participate in ongoing evaluation and collaboration with city staff and community partners.

10. Create meaningful contacts and connections with individuals experiencing homelessness.

Specific roles and tasks included, but are not limited to :

  • Obtaining identifying documentation such as birth certificates, social security cards, and Washington State Identification.
  • Establishing income such as Aged, Blind, or Disabled (ABD), Social Security, and assisting with opening bank accounts.
  • Assessing basic food such as Electronic Benefit Transfer cards (EBT) and providing knowledge of food banks and where to obtain meals.
  • Engaging in housing-based needs by connecting with the 211 Program and housing navigators to ensure their case remains open while working to find sober housing and transitional housing options, apartments, and or shelter options.
  • Obtaining health care insurance and ensuring those served have been or are seen by a health care provider.
  • Accompanying and arranging transportation of those served to appointments as needed.
  • Connect with other support systems such as mentors, friends, families', peers, recovery groups, volunteer organizations, job centers, and counselors. Page 9 of 27
  • Working alongside other community providers on care coordination to meet the needs of those served, such as coordinating with substance use and mental health care treatment providers to create care plans.
  • Advocating for participants with a wide variety of other suppliers.
  • Providing advocacy and support for participants within the criminal justice system, including court appearances, written communication, and follow-up with prosecutors and defense attorneys.
  • Assisting in holding other agencies accountable by advocating for clients served

ESSENTIAL SKILLS AND ABILITIES :

  • Demonstrated commitment to the vision, values and mission of the CMP project working with low income, court-involved, and homeless populations
  • Ability to advocate and effectively communicate and problem solve under pressure in high stress situations.
  • Identify gaps and barriers in available community resources and advocate for systemic changes.
  • Skills necessary to provide advocacy and support for participants within the criminal justice system including court appearances and written communication.
  • Ability to prioritize workload and daily activities and complete tasks in a timely and efficient manner.
  • Ability to set boundaries, resolve conflict and de-escalate issues.
  • Dependable, able to work under pressure; receptive to change, willingness to learn, cooperative approach to problem-solving.
  • Ability to flex schedule when necessary to accommodate special program needs.
  • Demonstrated ability to maintain confidentiality of personal health information under 45 CFR and alcohol and drug information under 42 CFR Part 2, as well as comply with all state laws and regulations and company policy.
  • Ability to use a keyboard and utilize electronic records and databases to efficiently enter necessary data and services documentation.
  • MINIMUM QUALIFICATIONS :

  • Bachelor’s degree in social work, psychology, sociology or other human services-related field, OR Associates degree in human services-related field and two years’ experience working directly with this population, OR Five or more years relevant experience working directly with this population.
  • Experience working with adults who are experiencing or who have experienced homelessness, have a mental illness and / or co-occurring substance abuse disorders.
  • Interest in or experience working with participants who are difficult to engage and refer to traditional programs.
  • Ability to communicate and work effectively with staff from various backgrounds.
  • Ability to work effectively with clients displaying a wide range of unpleasant and / or bizarre behavior..
  • Subscribe to the philosophy of cooperation and continuity across programs and of consideration and respect for others.
  • Understanding of harm reduction along with a demonstrated passion for serving individuals experiencing homelessness and co-occurring disorders required. Street outreach experience a plus.
  • Maintain current TB test and First Aid / CPR documentation.
  • Must have a valid Washington State driver's license and be able to travel on a daily basis, including transportation of HOST participants.
  • Must be able to pass a Washington State Patrol criminal background test.
  • No history of misuse of alcohol or other drugs for a minimum of one year prior to employment with Evergreen Recovery Centers.
  • Have no alcohol / drug-related charges for a minimum of one year prior to employment with Evergreen Recovery Centers.
  • Pass a post-offer, pre-employment drug test and agreement to agency’s drug free workplace policies, including compliance with random and / or for cause drug testing.
  • PREFERRED QUALIFICATIONS

  • Experience working with adults who are experiencing or who have experienced homelessness, have a mental illness and / or co-occurring substance use disorders.
  • Knowledge of de-escalation skills, crisis intervention & stabilization, and harm reduction strategies, preferred.
  • Current Washington State driver's license and insurable driving record.
  • Bi-cultural background / experience.
  • Bi-lingual in Spanish / English or other languages preferred.
  • Strong knowledge of relevant community resources and methods for accessing them.
  • PHYSICAL DEMANDS : Job requires regular sitting, standing, walking, stooping, bending, kneeling, crouching, reaching, pushing, pulling, twisting, and writing. While performing the duties of this job, the employee is regularly required to : sit; use hands to finger, handle or feel objects, implements or office equipment; talk and hear; stand and walk, reach with hands and arms; stoop or kneel. Employee must occasionally lift up to 20 pounds. Specific vision abilities required by this job include close, distance and color vision and the ability to adjust focus. Must be able to drive a vehicle. Language Skills : Ability to read and understand procedures, memos and forms. Ability to write clearly and cohesively and to record patient observations both orally and in writing. Reasoning Ability : Ability to assess situations, analyze options and apply procedures to interactive situations. Work Environment : The noise level in the work environment is usually moderate, with noise periodically generated by community members and police vehicles rather than machines.

    Special Work Conditions : Street outreach requires the ability to easily navigate city streets on foot. Requires the ability to walk and stand for several hours in a variety of weather conditions.

    Hourly non-exempt, working 40 hours per week with a combination of scheduled shift work, office hours and some rotating on-call hours. Work stations and offices will be located at Evergreen Recovery Centers on Airport Road at Everett / Lynnwood city boundaries and in Skagit County
