Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our team. In this role, you will be responsible for evaluating patients' physical conditions, developing personalized treatment plans, and guiding patients through rehabilitation exercises. You will work closely with geriatric patients in their homes. Your expertise in physical therapy will play a crucial role in helping patients regain mobility and improve their quality of life.
Duties : Conduct thorough evaluations of patients’ physical abilities and limitations. Develop and implement individualized care plans based on patient assessments. Administer therapeutic exercises and modalities to promote recovery. Monitor patient progress and adjust treatment plans as necessary. Collaborate with other healthcare professionals to ensure comprehensive patient care .Educate patients and their families about treatment options, exercises, and self-care strategies. Make sure PTA's Document patient progress accurately. Participate in discharge planning to ensure continuity of care post-treatment.
Skills : Strong knowledge of physical therapy principles and practices, including, orthopedics, and geriatrics. Knowledge of Oasis and Proficient in developing effective care plans tailored toward individual patient needs. We can train. Solid understanding of physiology as it relates to rehabilitation and the recovery processes. Excellent communication skills for interacting with patients, families, and multidisciplinary teams. Ability to motivate patients while providing empathetic support throughout their recovery journey. Strong organizational skills to manage multiple patient cases effectively.
