AlliedTravelCareers is working with Windsor Healthcare Recruitment Group, Inc. to find a qualified Licensed Clinical Social Worker in Green Bay, Wisconsin, 54311!

Pay Information

$3,200 per week

About The Position

  • Region : Oshkosh Market. rural geography in Wisconsin . Oshkosh team is

Fond du Lac through Appleton, whole west side of kind of around Lake Winnebago.

  • Unit Information :

    • Census? Average patients / day?

    Census of 30 patients in home / facility,

  • 4-5 visits / day (SW schedule themselves. So it's autonomous)
  • Minimum Years of Experience required? 1 year of experience SW. Highly prefer hospice experience and previous travel exp
  • Will you accept a first-time traveler? Yes, if they ve done hospice SW
  • Patient Types / Common Diagnoses :

    • End-of-life, no pediatric pts

  • Patient Ratios :

    • MSW : 1 : 35

  • Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and / or Insurance :

    • MSW

  • State License : LCSW
  • Driver's License
  • Skills required :

    • Hospice social work experience

  • Support within the Department :

    • 2 other SW in different markets available for additional support

  • Lead RN
  • Hospice Medical Director
  • Technology / Equipment :

    • EMR : EPIC Remote Client highly preferred

  • Floating : If yes, where?

    • Throughout region

  • Orientation (facility and unit) :

    • NEO

  • 2 days departmental- one week with hospice social worker usually (in person preceptor)
  • Scheduling :

    • Weekend rotation : None

  • On call? If so, what is that schedule? NA
  • Holiday Expectations : 1-2 holidays per contract, based on unit needs
  • RTO : Up to 5 days
  • Shift times : M-F 8-4 : 30p
  • Other notes :

    • Scrub Color : any, or business casual

  • Parking : Free
