Second Harvest Food Bank of Central Florida is a private, nonprofit organization that collects, stores and distributes donated food to more than 625 feeding partners in seven Central Florida counties : Brevard, Marion, Lake, Orange, Osceola, Seminole and Volusia.
In addition to providing food to our neighbors facing hunger, Second Harvest also transforms dozens of lives directly every year. Economically-challenged adults who graduate from our 16-week culinary training program are placed in 'better than minimum wage' jobs that set them and their families on a path to self-sustainability. Another important activity helps connect people who are eligible to receive SNAP food assistance benefits with the resources they need to access the program and get help.
Department Overview
Second Harvest Food Bank of Central Florida's (SHFB) guiding principle is that people facing hunger are at the center of all that we do. Our work to alleviate hunger is focused on increasing access to nutritious food and improving the diet quality of people experiencing hunger.
Work Hours
Monday-Friday 8 : 30am-5 : 00pm, hybrid work model with some night and / or weekend availability as needed. Up to 20% local travel.
Job Summary
Under the direction of the Health Initiatives Manager, this role will be charged with ensuring that patient care is at the highest standard to improve health outcomes in health initiative programs including but not limited to Bring Hope Home (BHH), Medically Tailored Meals (MTM), and other health and hunger initiatives.
Job Responsibilities
- Manage daily patient engagement through various communications channels for BHH, MTM and other health and hunger initiatives focusing on behavioral goal setting and patient centered support
- Provide patients with resources that meet their social determinant needs
- Responsible for managing and interpreting health outcomes for programmatic goals and grant projects
- Administer and monitor surveys and other feedback tools including but not limited to patient advisory committees and working groups
- Support the Registered Dietitian Nutritionist in providing services for chronic disease management and education
- Document patient safety concerns and participate in routine patient compliance meetings
- Oversee program level data management including daily input and outputs of data, Salesforce processes, enrollment process and develop and monitor data management buildouts
- Provide oversight of community partners by providing training, technical assistance, documentation, grant management and invoicing
- Oversee and generate programmatic reports for shareholders, grants, community partners, internal management and media / development
- Serve as point of contact for BHH and MTM Community Partnerships and / or community committees across service areas
- Implement health equity strategic objects for BHH and MTM
- Research and recommend healthcare partnership grant opportunities
- Oversee and evaluate eligibility requirements for BHH and MTM and program level quality assurance
- Other duties as needed
Requirements
Associate's degree or 2 years of experience in a Social / Human Services or Patient Advocate roleExperience cultivating and managing community and healthcare partnershipsComfortable communicating with people in crisis using trauma-informed care approachStrong interpersonal communication abilitiesProven success with collaboration and teamworkProficiency in planning, organizing and time managementProficient and accurate data entry skillsProven ability to manage multifaceted work, multi-task and thrive in a dynamic work environmentCritical thinking and problem-solving skills and ability to make quality independent decisionsStrong project and data management skillsDesired Skills and / or Certifications
