Responsibilities

Join an amazing team at a nationally recognized Top Place to Work in Healthcare! As a Home Care RN with UnityPoint at Home, you’ll provide one-on-one care to patients with total autonomy over your role. With a flexible schedule and independence, our home care nurses do the admit paperwork. This Nurse is doing the visit / care for the patient. They follow the care plan-similar to hospital bed side care but in home care setting and make it possible for patients to receive care where they’re most comfortable – in their homes.

The Home Care RN uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient’s place of residence. The RN contributes to the development of the home health plan of care. The RN delivers patient care directed by the physician as established in the home health plan of care that is consistent with clinical best practices and results in high quality, improved outcomes and exceptional patient experience.
