Job description

Case Management & Client Support

Assessment and intake interviewing

Ability to create individualized service or care plans

Crisis intervention skills

Knowledge of community resources

Communication & Documentation

Strong written and verbal communication

Clear and accurate documentation in case files

Ability to maintain confidentiality (HIPAA compliance in healthcare)

Coordination & Organization

Managing multiple cases at once

Scheduling, follow-up, and care coordination

Problem-solving and conflict resolution

Technical

Proficiency with case management software or EMR systems

Basic computer and administrative skills

Education

Bachelor’s degree in Social Work, Psychology, Counseling, Nursing, Human Services, or a related field.

Some roles prefer or require a Master’s degree (MSW, counseling, public health, etc.).

Experience

1–3 years of experience in :

Case management

Social services

Counseling or behavioral health

Community outreach

Healthcare navigation or care coordination

Experience working with at-risk populations is often highly preferred.
