Seeking a Tailored Care Management Supervisor to provide leadership, oversight, and support to Care Managers and Extenders delivering Tailored Care Management services.
This role ensures compliance with state and payer requirements, promotes high-quality, person-centered care, and supports staff development through supervision, training, and performance monitoring.
The TCM Supervisor plays a critical role in ensuring that care coordination meets the unique physical, behavioral health, and / or I / DD needs of individuals served.
Essential Duties and Responsibilities
- Provide clinical and administrative supervision to assigned Care Managers and Care Management staff, ensuring adherence to Tailored Care Management standards, policies, and procedures.
- Monitor caseload assignments, care plan quality, and service delivery to ensure person-centered, strengths-based approaches are consistently implemented.
- Oversee compliance with Medicaid Tailored Plan and NC DHHS requirements, including documentation, timeframes, and billing requirements.
- Support staff in identifying and addressing barriers to care, connecting individuals to medical, behavioral, social, and community-based resources.
- Conduct regular audits of care management notes, assessments, and person-centered plans for accuracy, completeness, and compliance.
- Facilitate staff onboarding, training, and ongoing professional development.
- Collaborate with internal leadership, external providers, and community partners to support integrated, whole-person care.
- Review and monitor key performance metrics (e.g., engagement, contact standards, outcomes) and implement corrective action plans as needed.
- Ensure crisis response protocols and Comprehensive Crisis Plans are completed and maintained appropriately.
- Maintain up-to-date knowledge of Tailored Care Management policies, value-based care initiatives, and best practices in care coordination.
- Participate in quality improvement initiatives and contribute to policy and program development.
Education and Experience :
Master’s degree in Social Work, Counseling, Psychology, Nursing, or related human services field.Active full or provisional licensure (LCMHC or LCSW) preferred; provisional licenses will be considered with supervision requirements.Minimum of 3 years’ experience providing care management, case management, or related behavioral health / healthcare services.At least 1 year of supervisory or leadership experience in a behavioral health, medical, or care management setting preferred.Knowledge, Skills, and Abilities :
LCSW / A, LCMHC / A, LCAS / A, or RNKnowledge of behavioral health, physical health, and / or I / DD populations.Strong understanding of Tailored Care Management model, Medicaid services, and North Carolina system of care.Ability to lead, coach, and develop staff while fostering a supportive, team-oriented environment.Excellent communication, organization, and problem-solving skills.Proficiency in electronic health records, Microsoft Office, and care management documentation systems.Commitment to person-centered, culturally competent, and trauma-informed care.Job Type : Full-time
