Social Work and Discharge Planning Assistant

LENOIR HEALTH AND REHAB in LENOIR, NC is seeking a qualified individual to join our growing team as the Discharge Planning & Social Work Assistant.

As the Discharge Planning & Social Work Assistant, you’ll play a crucial role in providing Care Beyond Compare by ensuring seamless transitions for our patients when they discharge from our facility back into the community. You’ll also collaborate closely with the Director of Discharge Planning & Social Work in addressing the emotional, social, and practical needs of our patients and helping patients and their families access resources they need for a fulfilling stay and safe discharge.
