About the Role:
Medadventures is seeking a dedicated and compassionate Physical Therapist to join our team in a skilled nursing facility (SNF) setting. This position offers the opportunity to work closely with residents to improve mobility, restore function, and enhance overall quality of life.
Responsibilities:

• Evaluate, plan, and implement individualized treatment programs for residents.
• Collaborate with an interdisciplinary care team to achieve patient goals.
• Document all treatments, progress notes, and discharge summaries according to facility and regulatory standards.
• Educate residents and caregivers on safe mobility, exercises, and home programs.

Qualifications:

• Active state Physical Therapy license.
• Previous experience in a SNF or inpatient rehab setting preferred.
• Excellent communication, organization, and clinical reasoning skills.
• Passion for patient-centered care and rehabilitation outcomes.
