Position Summary
The Community Health Outreach Advocate (CHOA) will directly assist the Director of Community Health and the Community Resource Center Manager in supporting the mission of Community Care Plan (CCP) to promote healthier communities. Through outreach and engagement activities, this position will assist community residents in linking to local resources that address Social Determinants of Health (SDoH) as well as improving access to healthcare. This position will engage with local and faith-based organizations representing CCP and, through engaging with community members, both improve their health and well-being while creating brand awareness of CCP in the community. Position duties will be primarily performed in the Lauderdale Lakes and surrounding areas in Broward County.

Work Schedule
Primarily remote within the region, with occasional hybrid work from the office. The Community Resource Center (CRC) is located at 3736 W Oakland Park Blvd, Lauderdale Lakes, FL 33311.
