Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience depending on the specialty.

License : Must have a valid license in hand.

Certifications : CPR, BLS

Must-Have :

Must have recent specialty experience in home health.

Must be willing to travel up to 75 miles from the home clinic.

Must be proficient in HomeCare HomeBase EMR and have used it within the last 6 months, or must complete a retraining course.

Must be comfortable with on-call rotations.

Description :

The position involves providing, coordinating, and directing home nursing care according to physicians' orders.

The nursing care is based on agency policies and procedures through the competent application of the nursing process.

Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses.

Daily responsibilities include traveling to patient homes and attending periodic office meetings.

It is essential to have reliable transportation, a current driver's license, and agency-required liability insurance.

The average number of patients per day ranges from 5 to 10.

Documentation and other activities hold different point values contributing to weekly productivity expectations.

All documentation must be completed within 24 hours of the patient appointment.

Delayed or incomplete documentation may impact billing and follow-up patient care.

There are no guaranteed hours with this contract.

Clinicians must accept all scheduled patients regardless of distance.

Failure to meet weekly productivity points may allow the opportunity to float to nearby clinics for additional hours.

This position requires strong communication skills and the ability to work independently in a home health setting.

Candidates must be prepared to manage various cases such as Start of Care, Wound Care, and Follow-up Visits.
