Brooksville Healthcare Center is seeking a compassionate and motivated Per Diem Physical Therapist (PT) to join our in-house rehabilitation team . Our facility offers subacute and long-term care rehabilitation , focusing on helping patients regain independence and improve their quality of life.

Responsibilities :

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide therapeutic interventions to improve mobility, strength, balance, and functional abilities.
• Monitor patient progress and adjust treatment as needed.
• Collaborate closely with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians.
• Maintain accurate and timely clinical documentation.
• Educate patients, families, and caregivers on home exercises and rehabilitation goals.
