We are searching for a talented LPN Telehealth - BAYLOR

Hours : 32hrs

Shift : Day shift, 8 : 00am - 4 : 30pm with weekend and holiday rotation

Location : Visiting Nurse Association - Fairhaven, MA

Under general supervision of the TL-Telehealth Program or designee, perform duties to support and in support of the Tele monitoring department in an effort to reduce re-hospitalizations and provide proactive symptom management while adhering to best practices. Address data packets that are sent via the telehealth system with patients, field staff, physicians and clinic staff. Triage phone calls and provide patient education as it relates to data collected via telephone as needed. Troubleshoot equipment via telephone with occasional need for home visits to patients for equipment set up and or problems. Provide video visits in compliance with patient care plan.

Compensation : Pay rate will be determined based on level of experience.
