The Health Services Director (LPN) will be an experienced dynamic leader with superb communication skills and a passion to serve. The Health Services Director serves as a liaison with hospital personnel physicians community organizations and other health related service agencies to provide quality care to our assisted living residents. Additionally this role is responsible for hiring training and managing a staff that is capable of providing superior care while maintaining the physical and emotional health of residents and in accordance with current federal state and local standards guidelines and regulations.

Responsibilities

Ensure that an initial assisted living resident assessment is completed prior to each residents move-in date and a comprehensive care plan is completed upon move-in

Ensure each assisted living resident is receiving the best possible care and programs to meet their individual needs

Work closely with all departments in the community to ensure assisted living residents are receiving all available services to enhance their quality of life
