Home Health- Physical Therapist

Butte Home Health is dedicated to providing compassionate and high-quality home health care services. We are seeking a skilled and motivated Home Health Physical Therapist to join our team. The ideal candidate will have experience in delivering physical therapy services in a home setting and a strong commitment to improving the quality of life for our patients.

Key Responsibilities:

• Evaluate and Assess: Conduct comprehensive assessments of patients' physical abilities and limitations. Develop individualized treatment plans based on assessment results.
• Develop Treatment Plans: Design and implement personalized therapy plans to improve patients' mobility, strength, balance, and overall functional independence.
• Deliver Therapy Services: Provide hands-on physical therapy services including exercises, manual therapy, and modalities to help patients achieve their goals.
• Patient Education: Educate patients and their families about their conditions, treatment plans, and strategies for maintaining and improving their physical health.
• Progress Monitoring: Regularly assess and document patients' progress, adjust treatment plans as needed, and communicate with other healthcare professionals regarding patient care.
• Collaboration: Work closely with physicians, nurses, and other healthcare providers to ensure coordinated and effective care.
• Compliance: Adhere to all regulatory and safety guidelines, maintaining up-to-date documentation and ensuring that all services are compliant with federal and state regulations.
• Maintain Equipment: Ensure that all therapy equipment is maintained and used correctly, and provide recommendations for any necessary equipment modifications.

Qualifications:

• Education: Degree in Physical Therapy from an accredited program.
• Licensure: Valid state licensure or certification as a Physical Therapist.
• Experience: Previous experience in a home health setting preferred but not required. New graduates are encouraged to apply.
• Skills: Strong clinical assessment and diagnostic skills, excellent communication and interpersonal skills, and the ability to work independently and manage time effectively.
• Physical Requirements: Ability to lift and assist patients as needed, and perform physical tasks associated with therapy services.
