Job description

Overview

Open Sky supports adults with developmental and intellectual challenges referred by the Department of Developmental Services. The organization aims to provide safe, nurturing homes that address individuals' personal needs and preferences. Individuals in Open Sky residential programs are encouraged to participate in community life through volunteering, classes, group outings, or joining health clubs. Our staff ensures the necessary supports and skills for a healthy, productive, and independent life in the community.

Position

DDS Residential Counselor (24 Hours) – West Brookfield Apts. Downstairs

Responsibilities

  • Assist in developing individualized, person-centered treatment plans and lead the daily implementation of plans.
  • Implement procedures that ensure health, safety, and well-being of individuals and maintain general cleanliness and safety of the physical site.
  • Assist individuals to access and / or provide transportation.
  • Review and complete all required documentation.
  • Support individuals diagnosed with a Developmental Disability to live as independently as possible within a group living environment, providing guidance, support, and activities of daily living as needed.
  • Advocate for individuals served and promote community integration and participation.

Qualifications

  • High School Diploma, GED, or equivalent required.
  • Valid Driver's License and acceptable driving record required.
  • Reliability, willingness to learn, and openness to new opportunities.
